Billing & Insurance
At EveresT Men’s Health, we know that understanding your insurance coverage and billing details is just as important as understanding your treatment plan.
Medical costs can feel complicated, but we believe you deserve clarity and peace of mind about your financial options. That is why we provide upfront, reliable information so you can make confident decisions about your care.
We are proud to be in-network with the following insurance providers:

If your insurance is one of these carriers, please bring your insurance card and a valid photo ID to your first appointment.
We also encourage you to confirm your benefits directly with your insurance provider, as coverage can vary depending on your plan. Knowing this information ahead of time helps avoid surprises and ensures a smoother visit.
Payment Options & Billing Support
For your convenience, we accept major credit cards as well as Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA). These options give you more flexibility in managing your healthcare expenses while making it easier to stay on top of your care.
We know billing questions can come up, and our administrative team is here to help. Whether you need an explanation of a statement, details about what’s covered, or guidance on payment options, we can walk you through everything step by step.

Medical Membership Options for Affordable Care
We understand that not every patient has in-network insurance coverage, and that’s okay. At EveresT Men’s Health, we have designed flexible membership programs to offer reliable access to high-quality care without the unpredictability of copays, deductibles, or out-of-network costs.
Many of our patients find that our memberships are not only simpler to manage but also more affordable in the long run. In fact, for some patients, membership costs may even be more cost-effective than what they would pay through insurance, including copays and deductibles.
We offer two main membership options:
- Concierge Medicine Membership: Provides access to general health care, routine monitoring, and wellness support.
- TRT Membership: Includes the benefits of concierge care, as well as additional services, such as TRT and other personalized care options.
Membership fees are billed monthly and can be paid with a Health Savings Account (HSA) or Flexible Spending Account (FSA) for added convenience. Our team is happy to answer any questions and help you determine which option may be best for your needs.
